Cactus Chiropractic is the DBA of Cactus Health and Wellness, PLLC. We are committed to protecting the privacy of our patients and providing clear information regarding access to health care records, regulatory oversight, and consumer complaints.

Pursuant to Texas Health and Safety Code § 181.105, the following information is provided to assist patients with requesting their health care records, contacting the appropriate licensing authority, and filing a consumer complaint.

Requesting Your Health Care Records

Patients may request a copy of their health care records maintained by Cactus Chiropractic.

To request your records, please submit a written request to our office. Your request should include:

Your full name
Date of birth
Current contact information
The date or approximate dates of service
A description of the records requested
Your preferred method of receiving the records, when applicable

Texas Board of Chiropractic Examiners

The Texas Board of Chiropractic Examiners regulates the practice of chiropractic in Texas and may receive complaints concerning the professional conduct or practice of licensed chiropractors.

Patients may contact the Board with questions or concerns regarding chiropractic care, professional conduct, or matters within the Board's regulatory authority.

Filing a Consumer Complaint

If you have a concern regarding your care, your health care records, or the handling of a records request, you may first contact Cactus Chiropractic so that we can attempt to address your concern.

You may also contact the appropriate regulatory authority directly.

For concerns regarding the professional practice or conduct of a Texas chiropractor, you may file a complaint with the Texas Board of Chiropractic Examiners.

Depending on the nature of the concern, you may also have the right to file a consumer complaint with the Texas Attorney General's Consumer Protection Division.
